<h2>Fire Concerns Prompt York International to Reannounce Recall of Gas Furnaces for Manufactured Homes</h2>
<p>WASHINGTON, D.C. &#8211; The U.S. Consumer Product Safety Commission and Health Canada, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p><b>Name of Product: </b>Coleman, Coleman Evcon and Red T Gas Furnaces for Manufactured Homes</p>
<p><b>Units: </b>About 223,600 in the U.S. and 2,400 in Canada  (previously recalled in November 2004)</p>
<p><b>Manufacturer: </b>Unitary Products Group (UPG), a division of York International Corp., of York, Pa.</p>
<p><b>Hazard: </b>The furnace can overheat and cause the heat-exchanger to crack and create openings that allow flames to be exposed. When this happens, drywall and other nearby combustibles are exposed to the flames, posing a fire and smoke hazard to consumers.</p>
<p><b>Incidents/Injuries: </b>The firm has received reports of 393 incidents, including some involving extensive property damage that could be related to these hazards, 366 of those reports were received after the November 2004 recall announcement. No injuries have been reported.</p>
<p><b>Description: </b>The recall involves Coleman, Coleman Evcon and Red T brand furnaces. The furnaces are silver with white access panels. Coleman,&#8221; &#8220;Coleman Evcon&#8221; and &#8220;Red T&#8221; brand names are located on the middle of the front access panel. The model number is written on a faceplate, found by removing both front access panels. The faceplate is found mounted on the left inside surface behind the lower panel. Models included in the recall are:</p>
<p><b>Sold by: </b>Nationwide between 1995 and 2000 as original equipment in manufactured homes and as replacement furnaces in manufactured homes.</p>
<p><b>Manufactured in: </b>U.S.A. between 1995 and 2000</p>
<p><b>Remedy: </b>Consumers should immediately stop using the furnace until it has been inspected and repaired. Consumers should contact UPG to schedule a free inspection and repair of any furnace involved in the recall.</p>

<p><b>Name of Product: </b>Overarching Floor Lamp</p>
<p><b>Units: </b>About 5,750</p>
<p><b>Importer: </b>West Elm, a division of Williams-Sonoma Inc., of San Francisco, Calif.</p>
<p><b>Hazard: </b>A short circuit can occur in the lamp&#8217;s wiring, posing a shock hazard to consumers.</p>
<p><b>Incidents/Injuries: </b>The firm is aware of at least 39 reports of short circuits in the lamp, including three reports of shock, one report of a minor burn to a consumer&#8217;s finger and two reports of minor property damage.</p>
<p><b>Description: </b>The Overarching Floor Lamp is 77 inches tall with a 19-inch diameter base and a curving arm that extends the lamp about 5 feet from the base. The arm and base are made of polished nickel. It was sold with a linen shade in the colors natural, white or charcoal. The lamp has a three-way on/off switch.</p>
<p><b>Sold by: </b>West Elm retail stores nationwide, West Elm catalogs and westelm.com from March 2011 through November 2011 for about $250.</p>
<p><b>Manufactured in: </b>China</p>
<p><b>Remedy: </b>Consumers should immediately stop using the lamp and return it to West Elm for a full store credit.</p>

<p><b>Name of Product: </b>Konica Minolta Printers</p>
<p><b>Units: </b>About 8,430</p>
<p><b>Importer: </b>Konica Minolta Business Solutions U.S.A. Inc., of Ramsey, N.J.</p>
<p><b>Hazard: </b>The printers can short circuit and overheat during use, posing a fire hazard.</p>
<p><b>Incidents/Injuries: </b>The firm is aware of two reports of the printers overheating. No injuries have been reported.</p>
<p><b>Description: </b>This recall involves four desktop models of laser color printers: Magicolor 4750DN, Magicolor 3730DN, Bizhub C35 and Bizhub C35P. Model numbers are located on the plate attached to the side of the printer as well as the bottom of the front door. The printers are dark and light gray in color and have &#8220;Konica Minolta&#8221; printed on the top of the front door.</p>
<p><b>Sold at: </b>Various value-added resellers, direct retail sales and authorized Konica Minolta dealers from June 2010 through March 2011 for between $900 and $3,500.</p>
<p><b>Manufactured in: </b>China</p>
<p><b>Remedy: </b>Consumers of Magicolor 3730DN and 4750DN should stop using the printers immediately and contact Konica Minolta to schedule a free replacement. Consumers of Bizhub C35 and C35P will be visited by an authorized service agent for repair and replacement of the faulty component.</p>

<p><b>Hazard: </b>The welding joints on the legs can break, posing a fall hazard to consumers.</p>
<p><b>Incidents/Injuries: </b>The firm has received four reports of ottoman legs that have broken, including two reports of minor injuries.</p>
<p><b>Description: </b>This recall involves fold-out sleeper ottomans with a brown slipcover. The ottoman unfolds to three cushions on a black, steel frame. Velcro straps secure the cushions to the frame when in the unfolded sleep position. A tag with the manufacturer&#8217;s registration number &#8220;VA18265(CN)&#8221; is attached to one of the three cushions.</p>

<h2>HP Recalls Fax Machines Due to Fire and Burn Hazards</h2>
<p>WASHINGTON, D.C. &#8211; The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p><b>Name of Product: </b>HP fax 1040 and 1050 machines</p>
<p><b>Units: </b>About 928,000 in the U.S. and 240,000 in Canada and Mexico</p>
<p><b>Importer: </b>Hewlett-Packard Co., of Palo Alto, Calif.</p>
<p><b>Hazard: </b>The fax machines can overheat due to an internal electrical component failure, posing fire and burn hazards.</p>
<p><b>Incidents/Injuries: </b>Hewlett-Packard is aware of seven reports of fax machines overheating and catching fire, resulting in property damage, including one instance of significant property damage and one instance of a minor burn injury to a consumer&#8217;s finger. Six incidents were reported in the U.S. and one in Canada.</p>
<p><b>Description: </b>This recall involves HP Fax 1040 and 1050 models. The HP logo and the model number are printed on the front of the fax machine. The fax machines are dark gray and measure about 11 inches high x 14 1/2 inches wide.</p>
<p><b>Sold at: </b>Electronics, computer and camera stores nationwide, and online at www.shopping.hp.com and other websites from November 2004 through December 2011 for between $90 and $120. Some of the recalled fax machines were replacement units for a previous recall involving HP fax model 1010 in June 2008.</p>
<p><b>Manufactured in: </b>China</p>
<p><b>Remedy: </b>Consumers should immediately stop using the recalled fax machines, disconnect them from the electrical outlet and contact HP for a rebate on the purchase of an authorized replacement HP fax machine or a partial rebate of certain HP ink jet printers.</p>

<p><b>Name of Product: </b>Lush Life® power strips</p>
<p><b>Units: </b>About 7,200</p>
<p><b>Importer: </b>The Container Store Inc., of Coppell, Texas</p>
<p><b>Hazard: </b>The power strips have undersized wiring, and the wiring and plastic strip fail to meet the requirements for fire resistance, posing a fire hazard.</p>
<p><b>Incidents/Injuries: </b>None reported</p>
<p><b>Description: </b>This recall involves three designs of brightly-colored, six-outlet power strips with model number 8001-6 embossed on the back. Power strip designs include Pop Power Strip in Scroll (black with white scroll), Dot (blue with orange, white and lime green dots and a lime green cord), and Zebra (black and white zebra stripes). The power strips have three-foot power cords.  Lush Life® power strips by design is printed on the products packaging.</p>
<p><b>Sold at: </b>The Container Stores nationwide and on the firms website www.containerstore.com from October 2011 through December 2011 for about $15.</p>

<p><b>Hazard: </b>The candle warmers can catch fire because the warming bowl is positioned too close to the tea light candle, posing fire and burn hazards.</p>
<p><b>Incidents/Injuries: </b>The firm has received one report of the Snowmen candle warmer catching on fire causing a minor burn injury to a consumers hand.</p>
<p><b>Description: </b>This recall involves holiday-themed tea light candle warmers in the shapes of snowmen and gingerbread houses. A warming bowl with wax fragrance tablets is suspended above the tea light candle. The Snowmen model has SKU/UPC 000016005396 and the Gingerbread house model has SKU/UPC 000016005372 printed on a label on the bottom of the product. The tea light candle warmers are about eight inches tall.</p>
<p><b>Sold exclusively at: </b>Christmas Tree Shops in the Northeast, Mid-Atlantic and Midwest from November 2011 through December 2011 for about $8.</p>

<p><b>Hazard: </b>An electrical short circuit can occur in the lights internal wiring, causing smoke and posing a fire hazard.</p>
<p><b>Incidents/Injuries: </b>The firm has received one report of the product causing smoke and a burning smell.  No injuries or property damage have been reported.</p>
<p><b>Description: </b>The recalled Hurricane Style Light is a circular lamp with a white metal basket and  moveable handle for hanging or carrying at the top.  It has a colored glass insert in blue, pink, or green and uses a mini light bulb. The model number DML0593 and SKU/UPC number 000015806710 are printed on the price tag attached to the product.</p>
<p><b>Sold at: </b>Christmas Tree Shops in the Northeast, Mid-Atlantic and Midwest regions from December 2010 through November 2011 for about $7.</p>

<p><b>Name of Product: </b>LED Flashlight and Battery Set</p>
<p><b>Units: </b>About 41,000</p>
<p><b>Importer: </b>BJs Wholesale Club Inc., of Westborough, Mass.</p>
<p><b>Hazard: </b>The flashlights can heat up, smoke or melt when turned on, posing fire and burn hazards.</p>
<p><b>Incidents/Injuries: </b>BJs has received two reports of incidents with the flashlights, including one unit overheating and one unit burning and making a loud noise. One minor injury to a consumers hand has been reported.</p>
<p><b>Description: </b>The recall involves Superex SAFETO GO LED flashlight and battery sets. The flashlight sets include one 9.5-inch flashlight with 10 LEDs, two 7.5-inch flashlights with seven LEDs and two 6-inch flashlights with five LEDs. The sets also include seven D size batteries and four AA size batteries. The flashlights in the sets are made of either red or black plastic with black or gray rubber around the handles and light bases. The words Superex, SAFETO GO Family Pack, and Bright White LED Flashlights appear on the front of the packaging. The UPC 0-56986-01008-3 is printed on the back of the packaging along with the SKU number 700935.</p>
<p><b>Sold at: </b>BJs Wholesale Clubs from July 2011 through December 2011 for about $15.</p>
<p><b>Manufactured in: </b>China</p>
<p><b>Remedy: </b>Consumers should immediately stop using all of the flashlights and return them to any BJs Wholesale Club for a full refund. Consumers should properly dispose of the batteries before returning the flashlights to the store.</p>

<h2>Expandable Lunch Boxes Recalled by California Innovations Due to Freezer Gel Pack Ingestion Hazard</h2>
<p>WASHINGTON, D.C. &#8211; The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.</p>
<p><b>Name of Product: </b>Expandable Insulated Lunch Box with Freezer Gel Pack</p>
<p><b>Units: </b>About 248,000</p>
<p><b>Importer: </b>California Innovations Inc., of Toronto, Canada</p>
<p><b>Hazard: </b>Gel that contains diethylene glycol and ethylene glycol can leak out of damaged freezer gel packs, posing a poisoning hazard if ingested by children or adults.</p>
<p><b>Incidents/Injuries: </b>The firm has received two separate reports of a dog chewing and ingesting gel from the gel pack. One dog reportedly died from ingestion of the gel. The other was reported to have received treatment and has recovered.</p>
<p><b>Description: </b>The recalled product is a Ci Sport three-piece, expandable, insulated lunch box set, which includes the lunch box, aluminum bottle and the freezer gel pack. The lunch box of the recalled set has the code &#8220;1-61731-99-57&#8243; printed on one of the two white labels that are sewn under the white fastener inside the main compartment. The lunch box is 8 inches wide, 5 inches deep and 10.25 inches high. It is made of vinyl, polyester nylon and crushed nylon and has a logo with the words &#8220;Ci Sport&#8221; attached to the upper left corner. The lunch box has a carrying handle on the top and a mesh carrying pouch for the aluminum bottle on the side. The aluminum bottle measures 2.75 inches in diameter and 7.5 inches tall. The gel pack is a 6-inch by 4-inch transparent plastic pouch filled with blue liquid gel. The words &#8220;Cryofreeze&#8221; and &#8220;Ice Pack/Hot Pack&#8221; are printed in white letters on the front of the pouch. The lunch box was available in four colors: navy blue, red, black and denim blue.</p>
<p><b>Sold at: </b>Costco Wholesale Clubs, Leon Korol and Cost U Less stores between May 2007 and September 2008 for about $10.</p>
<p><b>Manufactured in: </b>China</p>
<p><b>Remedy: </b>Consumers should immediately stop using the gel packs and dispose of them according to federal, state and/or local regulations. It is recommended that consumers contact their local waste disposal authority for instructions. Consumers may return the lunch box set to Costco for a full refund or may receive a $5 cash refund for the gel pack only by contacting California Innovations.</p>
